PerioGard is a brand name for a mouthwash that contains chlorhexidine gluconate as its active ingredient. Chlorhexidine is the actual antiseptic compound used to reduce bacteria in the mouth. So, PerioGard and chlorhexidine are not exactly the same, but PerioGard contains chlorhexidine.
