The most alarming side effect of tamoxifen is an increased risk of developing blood clots, such as deep vein thrombosis or pulmonary embolism. Additionally, it can slightly increase the risk of endometrial (uterine) cancer. These side effects are serious and require immediate medical attention if symptoms occur.
