The main downsides of Omnipod are less about “it’s bad” and more about the tradeoffs of a tubeless insulin pump.
1. Risk of insulin under-delivery or over-delivery
Like any pump, a Pod problem, blockage, software issue, or communication failure can interrupt insulin delivery. Omnipod’s own safety information warns that hardware defects, software glitches, and Pod failures can lead to hypoglycemia, hyperglycemia, or diabetic ketoacidosis (DKA).
2. You still have to monitor glucose carefully
Omnipod 5 depends on accurate CGM data in automated mode. If the CGM reading is wrong, the system can give too much or too little insulin. Omnipod specifically warns that inaccurate sensor values can lead to hypo- or hyperglycemia.
3. Skin irritation, adhesive reactions, and site infections
The Pod sticks to the skin for up to 72 hours, so some people get redness, itching, irritation, bleeding, pain, or infection at the site. Omnipod safety information tells users to watch for infection and skin irritation.
4. Pod bulk and placement issues
It is tubeless, which many people like, but the Pod is still a visible lump on the body. It can get knocked off by clothing, doorframes, sports, sleeping position, sweating, or adhesive failure.
5. You must replace Pods often
Pods usually last up to 3 days / 72 hours, and some people need changes every 48 hours depending on insulin use. This means ongoing supply management, refills, travel backups, and waste. Omnipod’s prescribing information notes Pod changes every 72 or 48 hours and that some users need a 48-hour change frequency.
6. Cost and insurance coverage can be a barrier
Omnipod is prescription-based and often goes through pharmacy coverage, but out-of-pocket cost varies by insurance. Omnipod states that exact coverage depends on the patient’s plan.
7. Device/app/phone limitations
Omnipod 5 uses an app/controller and only authorized compatible devices. Omnipod warns not to use unauthorized devices because it could interrupt insulin delivery. It also requires compatible CGM supplies for automated mode.
8. Alarms and troubleshooting burden
You need to respond to alarms, carry backup supplies, keep the controller/phone nearby, and know what to do if a Pod fails. Omnipod warns users to keep an emergency kit and carry Pod-change supplies.
9. Not suitable for everyone
Omnipod 5 is not recommended for people who cannot monitor glucose as advised, cannot maintain contact with their healthcare provider, cannot use it as instructed, or cannot hear/see alerts and alarms. SmartAdjust also should not be used in certain populations, including people needing less than 5 units of insulin per day, because safety has not been evaluated there.
10. Recalls or lot-specific issues can happen
In March 2026, FDA posted an Insulet correction for specific Omnipod 5 Pod lots due to a possible small tear in internal tubing that could cause insulin to leak inside the Pod rather than be fully delivered, potentially causing high blood glucose and DKA. Insulet said other lots and Omnipod products remained safe to use, and affected users should check lot numbers and replace affected Pods.
The biggest medical downside is this: because Omnipod delivers rapid-acting insulin only, delivery failure can cause blood sugar to rise faster than with long-acting insulin injections. Seek urgent medical help if you have high glucose with moderate/large ketones, vomiting, deep/rapid breathing, severe weakness, confusion, or signs of DKA.
