Vancomycin begins acting against susceptible bacteria after the first dose, but symptom improvement is not immediate.
- Oral vancomycin for C. difficile: Diarrhea and abdominal symptoms often begin improving within 2–3 days, though sometimes it takes several days. Continue the entire prescribed course—commonly 10 days—even when you feel better. Mayo Clinic
- IV vancomycin for serious infections: Improvement in fever, pain or other symptoms is generally assessed over the first 48–72 hours. Bloodstream, bone, heart-valve or other deep infections may take considerably longer to resolve.
Contact your healthcare provider if you are not improving after 2–3 days or are worsening. For C. difficile, seek urgent care for severe abdominal swelling or pain, inability to drink, reduced urination, confusion, blood in the stool or rapidly worsening diarrhea. Do not stop vancomycin early without medical direction.
